  • The Quality of Care Star Ratings Analysis report allows facilities to compare their quality measure rates against the Home Health Compare national rates. The report will be updated to reflect the new national rates published by CMS.
ABILITY OPTIMIZE Version 2.3.12, Release Date: September 17, 2019

We are pleased to announce the following enhancements to your ABILITY OPTIMIZE application.

OASIS database updates in response to CMS’ Home Health Prospective Payment System (HH PPS) Grouper v8219
  • The required update will be applied to the OASIS database to reflect the new diagnosis codes (FY20 ICD-10-CM), effective October 1, 2019 for Diagnostic Group assignments.
    • Existing code set will be updated to remove deleted diagnosis code(s) and update revised diagnosis code descriptions.
Update to Verification Rules
  • Remove verifications no longer applicable beginning October 1, 2019.
New assessment alert
  • A warning flag will trigger when a fall risk assessment has not been completed for a chairfast patient that is marked as needing assistance with dressing and toileting.

  • New Measures:

    • Application of Percent of Residents Experiencing One or More Falls with Major Injury
    • Application of Percent of Long-Term Care Hospital Patients with an Admission and Discharge Functional Assessment and a Care Plan that Addressed Function

The three new quality measures will be available to select and view for episodes of care that start on or after January 1, 2019.

  • Note: On the Patient Detail and Comparison reports, when selecting the Home Health Compare – All parameter, you must then select the measure you wish to view from the Measure drop-down.

    • Patient case volume, plus asterisks to highlight groups representing fewer than 20 patient cases, to easily identify which groups have more of an impact on your organization’s score and which may be too small to be reflective of true performance
  • If you have multiple CCNs and run the comparison report by agency, you will now see the agency name in addition to the CCN
  • Export functionality has been added; now you can create a PDF version or save the data in Excel

    • More date parameter options:
      • Use the assessment date (M0090) from the start (SOC/ROC) or end (transfer/discharge) assessment for the patient episode of care; the measure date (which aligns with the time frame during which the patient case will be included in the outcome results); or the assessment modification date (when either assessment in the patient case of care was last updated and submitted to ABILITY)
      • Option to select a date range based on specific days, rather than full months only
    • Limit the list to just patient care cases where a specific outcome or process measure was met or unmet, or include all cases, regardless of outcomes status

  • Drill into a specific measure to see additional patient level detail on scores within each assessment.
  • See who may not or have met criteria for that measure (red, green).
  • View whether patient scores have declined, stabilized, improved (red, yellow, green).
Comparison Report*
  • Compare performance via a side-by-side analysis of quality measures by clinician, team, branch, agency-level, primary diagnosis or payer
  • Compare quality measures to agency norms
    • 5 points higher than agency norm will display green
    • 5 points lower than agency norm will display red
    • Within 5 points of agency norm displays grey
