In today's complex and often fragmented healthcare delivery system providers will need the ability to access the complete set of healthcare information across the system for their patients. One of the emergent properties of a truly connected healthcare community is comprehensive clinical awareness of knowing where different parts of a patient's record are across the system and the ability to easily and securely share that information with one another.

Clinical Awareness Services
Too often, our caregivers are being asked to diagnose and treat patients using incomplete clinical information because patient information is fragmented across the healthcare system. Some information is at the doctor’s office, some at the hospital, some at the lab or pharmacy. How can we expect our physicians to come up with the best care plan without all of the patient’s clinical information?
At ABILITY, we securely see clinical information flow through our network from over 3,000 hospitals and tens of thousands of other providers across the country. Not only do we see patient information fragmented across different provider locations, but we see fragmentation within healthcare sites, as well. This happens when a patient goes back to a hospital, for example, and for whatever reason the hospital registration process doesn’t find the patient’s previous medical record, and instead creates a new one. Maybe the patient’s last name changed, or the address is different, or there was a typo in the social security number. Now, that patient has two (or more) medical records at the hospital, with their information fragmented across the set.
The industry average for this “same-site” fragmentation, or duplication of medical records, is 5-7%. That doesn’t sound so bad, but here’s what 5% means:
5% means that 1 in 20 patient visits results in care being provided with incomplete clinical information. 1 in 20. Every hospital across the country treats more than 20 patients each day, which means every day at every hospital there are patients in beds being treated with incomplete clinical information. This is a serious patient care issue, and likely the single most addressable patient safety issue within every hospital today.
With ABILITY, our providers get affordable, automated notifications of when fragmentation occurs within their site – while the patient is still in-house, so care can be adjusted accordingly. And, even better, our network services help registration and scheduling staff see previous patient records, so duplicate records shouldn’t be created. We help you diagnose the problem, and prevent it from reoccurring.
Further, our providers have the ABILITY for complete clinical awareness of where patient information is fragmented across the healthcare system. This means that as a patient presents at your care location, our automated network brings back a secure list of which other locations on the network also have clinical information for this patient, so a complete care record can be constructed. Like Google for docs – we help our caregivers get the complete picture on their patient’s clinical information.
With ABILITY, caregivers can construct the most efficient, safe, and effective care plans for their patients. Not only is this critically important for patient safety and overall care, but it directly reduces the number of duplicative and unnecessary clinical tests and procedures – making the overall patient care experience better and more affordable. Everybody wins with ABILITY.
